The Office of Student Integrity cannot release disciplinary information unless the student in question signs a Release of Information form. Please access the link below for that form.

In order for the Office of Student Integrity to process internal institution discipline checks, the Background Check form must be completed by the appropriate department designee. Please click the link below to access the instructions and the form.
